The Health Care Navigator II supports veterans in accessing VA and community healthcare services through coordinated, veteran centered care. This role partners closely with veterans, providers, and interdisciplinary teams to remove barriers to care and improve health outcomes, particularly for individuals experiencing homelessness or complex health needs.
